New Compendium Question

One new four-part question will be included in the Compendium. This question addresses the production and mailing of quarterly tax and wage reports. Occasionally questions have arisen about the possibility of including information for all employers in the quarterly statements rather than incurring the cost of an additional mailing to all employers. This might be accomplished by including this information in the quarterly tax and wage reports. The feasibility of including additional information is based upon the way in which the State produces the quarterly tax and wage reports. The following questions address only to the quarterly tax and wage reports:

1. Does your State use a self mailer?

   If "Yes", do you have the capability of including additional messages by overtyping on the form?

    

2. Does your State use a machine to stuff forms?

   If "Yes", can additional materials be included in the envelope?

    

3. Does your State hand stuff forms?

   If "Yes", can additional materials be included in the envelope?

    

4. Does your State do four quarterly mail outs?

   If "No", does your State mail all four quarters at once?
