Announcing the Extension of the Office of Management and Budget (OMB) Clearance for the ETA – 9000 Report on Internal Fraud Activities |
Purpose. To inform states that OMB has approved the extension for the collection of the ETA-9000 Report on Internal Fraud Activities.
References. ETA Handbook No. 376, Guidelines for Internal Security in Unemployment Insurance Operations.
Background. Form ETA-9000 provides information on the state workforce agencies (SWAs) internal fraud activities in the unemployment insurance program. SWAs submit the ETA-9000 report to the U.S. Department of Labor annually. The Fiscal Year 2002 report, ending September 30, 2002, was due on November 20, 2002.
OMB Approval. This reporting instruction has been approved by the Office of Management and Budget (OMB) under the Paperwork Reduction Act of 1995, OMB approval number 1205-0187, expiration date: 02/28/2005. Persons (SWAs), are not required to respond to this collection of information unless it displays a currently valid OMB control number.
Disclosure Statement. Based on information obtained from SWAs, it will
take an estimated three hours to prepare, complete and electronically submit an ETA-9000
report to the National Office. Respondent’s obligation to furnish this information is
mandated by Section 303 (a)(6), Social Security Act.
If electronic transmission of the report is not available, then and only then should the
report be sent in by hard copy. In such cases, one copy of the report should be sent to
the appropriate Regional Office; the original and one copy should be sent to: U.S.
Department of Labor, Employment and Training Administration, Office of Workforce Security
(DFAS), Room S-4231, Frances Perkins Building, 200 Constitution Avenue, N.W., Washington,
D.C., 20210.
