Instructions and Guidelines for the Annual Summary Report of Services provided to Migrant and Seasonal Farmworkers (MSFWs) |
Purpose. To transmit guidelines for the preparation and submission of the Annual Summary Report of Services provided to MSFWs.

Background. Federal regulations require each State Monitor Advocate to prepare an annual summary report reflecting an assessment of services provided to MSFW customers. TEN No. 1-03 provides guidelines to State Workforce Agencies (SWAs) on the preparation of quarterly MSFW reports, including the completion of the quarterly report using ETA Form 5148. Monitor Advocate annual summary reports are considered narrative attachments to the fourth quarter report.
State Annual Reports. In order to maintain uniformity in state reports, the attachment is the recommended outline for states to use as a guide in the preparation of the annual summary report.
OMB Approval. The reporting requirements for ETA Form 5148 are approved by OMB according to the Paperwork Reduction Act of 1995 under OMB approval No. 1205-0039, expiration date, June 30, 2004.
Action Required. State Administrators are requested to:
a) Provide the attached information to appropriate state staff; and
b) Forward the Annual Summary Report of Services Provided to MSFWs for Program Year (PY) 2002 to their Regional Office within 30 days of this issuance preferably in an electronic format. A copy should also go to the National Monitor Advocate, Erik Lang (lang.erik@dol.gov).
