Confidential Data Request Form for the North American Free Trade Agreement Transitional Adjustment Assistance (NAFTA-TAA) Program

  1. Purpose. To provide a copy of ETA Form 9043, NAFTA Transitional Adjustment Assistance Confidential Data Request.

  2. References. The Trade Act of 1974, as amended; and the Paperwork Reduction Act of 1980, as amended.

  3. Background. ETA Form 9043 is approved by the Office of Management and Budget (OMB) in accordance with the Paperwork Reduction Act of 1980, as amended, OMB approval number 1205-0339, expiration date August 31, 2004.

  4. Action Required. State administrators are requested to distribute the approved form to appropriate state staff.

  5. Inquiries. Inquiries should be addressed to the appropriate Regional Office.
