Petition for Trade Adjustment Assistance Form Including a Spanish Version

 

  1. Purpose. To provide copies of ETA Form 8560, Petition for Trade Adjustment Assistance, and the Spanish version, ETA Form 8559, Solicitud de Asistencia Para Ajuste Ocupacional.

  2. OMB Approval. ETA Form 8560 and ETA Form 8559 are approved by the Office of Management and Budget (OMB) in accordance with the Paperwork Reduction Act of 1980; OMB approval number 1205-0192; expiration date 07/31/2003.

  3. Action Required. State Administrators are requested to distribute these approved forms to appropriate State staff.

  4. Inquiries. Inquiries should be directed to the appropriate Regional Office.

  5. Attachments.
